Examining Acute Abdomen in a COVID-19 Positive Patient

The current COVID-19 pandemic has gripped the entire world putting our health care system at great stress and unimaginable demand. All surgical procedures on all suspected COVID-19 patient have been postponed until confirmed infection clearance. But there are emergencies which cannot be deferred. We have to gear up to face the challenge. SGT medical college and Research Institute has taken measures to fight the pandemic.

We have created a scenario of acute abdomen in a COVID positive patient and showed how it should be tackled. Whenever a COVID positive patient comes to an emergency with acute abdominal pain one of our team members attends him taking all standard precautions and wearing PPE.
While entering the room he sanitises his hands, takes a detailed history and examines him. After examination, he again sanitises his hands. He briefs the patients and takes him to his confidence after answering all his questions and allays all his fears about COVID. He makes note of clinical history, all examination findings and carries out the required investigations. It is to be noted that Minimal staff should be involved and only one member attends him initially and also subsequently. If a large number of surgeons are exposed to infected patients, the possibility for them to become infected and require self-isolation is real and could potentially result in a dangerous shortage of expertise within surgical teams. Resource usage should be carefully considered when planning scheduled procedures, particularly with regard to materials, staff, devices, intensive care beds, blood components, etc. He will take decision regarding future management of the patient. Surgery will be carried out if it is a really a life threatening situation.

If surgery is required it is carried out taking all standard precautions in a dedicated COVID operation theatre.
